DCT will prepare the form(s) required to obtain the passport you request based on the information you provide. A step-by-step guide is provided for obtaining an appointment, if applicable, directions on what to do next, and exactly what is expected from you.

Form & Document Preparation
DCT will prepare and provide everything in the Form Preparation package along with obtaining the certified copy required of the birth certificate and/or marriage record or any other additional vital records required to complete your application.

Need your form quickly? No worries. With our Expedite Service you choose how fast you receive your completed form(s).
*Turnaround time is dependent on how quickly the client provides appropriate information to complete the appropriate forms. If vital record procurement is selected, the turnaround time of the applicable agency to provide the vital record has no expedited turnaround times and is out of the control of DCT.
